Personal Directive and Enduring Power of Attorney Workshop

This workshop explains the importance of having a Personal Directive and Enduring Power of Attorney in place and will be facilitated by a Healthy Aging Social Worker who is also a Commissioner of Oaths.
Personal Directive – a document to name someone to make personal decisions for you in case you become incapable due to illness or injury.
Enduring Power of Attorney – a document to name someone to make financial decisions on your behalf if you become incapable due to injury or illness.
During this workshop, the participants will:
– Learn the importance of having these documents
– Receive step-by-step instructions on how to fill out the paperwork
– Work with the Healthy Aging Social Worker to complete the forms properly
– Have the finished papers witnessed and signed, making them official
