Our PCN offers the following programs and services:

Patient’s Medical Home

The Patient’s Medical Home (PMH) is a family practice defined by its patients as the place they feel most comfortable – most at home – to present and discuss their personal and family health and medical concerns.

It is where a team or network of caregivers, including registered nurses, registered dietitians,  exercise specialists, behavioural health consultants, mental health therapists, and other health professionals work together with the patient’s family doctor to provide and coordinate care.

COVID-19

Our PCN makes the safety of our patients, staff and the community our top priority. While in the PCN office, all patients and guests are encouraged to wear a medical face mask. All staff wear medical masks. Infection control takes place multiple times a day in both patient spaces and staff areas.

Appointments can be offered either in-person or virtually.
