Personal Directive & Enduring Power of Attorney

February 8 @ 5:30 pm - 8:00 pm

This workshop explains the importance of having a Personal Directive and Enduring Power of Attorney in place and will be facilitated by a Healthy Aging Social Worker who is also a Commissioner of Oaths.

Personal Directive – a document to name someone to make personal decisions for you in case you become incapable due to illness or injury.
Enduring Power of Attorney – a document to name someone to make financial decisions on your behalf if you become incapable due to injury or illness.

During this workshop, the participants will:

  • Learn the importance of having these documents
  • Receive step-by-step instructions on how to fill out the paperwork
  • Work with the Healthy Aging Social Worker to complete the forms properly
  • Have the finished papers witnessed and signed, making them official

Guidelines for registration:

  • Participants must be 18 years of age or older
  • Participants must register themselves
  • Participants MUST be able to understand and communicate in English. No interpreters are permitted as the liability of creating legal documents through an interpreter is too high.
  • Participants must be able to fill in their own PD and EPA templates under the direction of the group facilitator. A support person can assist at the direction of the participant with their consent.
  • If a participant has a hearing impairment, cognitive deficits or other physical or psychological barriers that make it a challenge to function within a group environment this is not the appropriate workshop.
  • No more than 2 people from the same family can attend the same group. This reduces the risk of abuse and coercion and an imbalance of group dynamics.
  • A support person is not required. If your patient wants to bring one, the support person must call in and register themselves. Experience shows the majority of support people want to complete their own documents once the group starts.
  • It is IMPERATIVE to bring photo ID to the appointment. EPA documents cannot be completed without a photo ID.

To register for this workshop, call our clinical administrator Ronda at the PCN to complete the registration process. 780-986-6624.
