Advance Care Planning

Documenting your preferences for your health care and personal matters helps to prepare your loved ones for the future. 🤍 This is called Advance Care Planning. You can make sure the people you trust know what to do if there is ever a time that you are unable to make or communicate decisions yourself.

There are lots of free and low-cost community resources to help you with this process, including your health care team. A PCN social worker can help answer your questions and provide you with guidance around completing these documents. Our PCN also offers a Personal Directive & Enduring Power of Attorney workshop, where you leave with completed documents.
