LBD PCN Programs

The Team-Based Care Program provides support to patients with a variety of concerns in both a centralized and decentralized setting.
PCN healthcare providers offer in-person and virtual workshops and classes to our patients and the public. Workshops and classes are free and don't require referral from your family physician, however registration is required. Both live group and on-demand classes are available.
Our PCN offers FREE Smoking Cessation assistance.
The Social Worker will provide comprehensive social work services to patients within the Leduc Beaumont Devon Primary Care Network.
Specialized geriatric assessment service for seniors who have chronic, complex health issues.
The LBD PCN currently offers specialty clinics to our patients in psychiatry and musculoskeletal concerns (MSK Clinic).
The LBD PCN has a team of 5 Referral Coordinators who assist member physician clinics in scheduling and tracking appointments with specialists, outpatient clinics and other community agencies.
Nurse practitioners (NPs) are registered nurses with graduate education and training in advanced clinical practice.
Several PCNs have partnered with local facilities in a unique venture where local family physicians and their health care teams prescribe physical activity to patients.
Devon and Calmar clinics offer ours outside of normal business hours.
